CAMO.A.125 requires the organisation to report to the competent authority and the organisation responsible for the type design any condition of an aircraft or component that hazards flight safety, as required by Regulation (EU) No 376/2014.
What it means in practice
The organisation must have a system for identifying reportable occurrences and submitting them to the competent authority and the type certificate holder within the required timeframes. Reportable occurrences include any failure, malfunction, defect, or condition that has endangered or may endanger the safe operation of the aircraft. Reports must also be submitted for incidents identified during continuing airworthiness management activities, such as discrepancies found during airworthiness reviews.
The reporting obligation extends to occurrences identified through record reviews, not only during physical inspections. If the CAMO identifies through documentation analysis that a mandatory inspection was missed or an airworthiness directive was not correctly implemented, this constitutes a reportable occurrence.
Key requirements
Reports must be submitted within 72 hours of the organisation identifying the occurrence, using the format and system established by the competent authority. The organisation must maintain internal records of all occurrences reported and any follow-up actions taken. A just culture environment must be fostered to encourage reporting without fear of punitive action against individuals.
Common compliance gaps
Under-reporting is a widespread compliance gap, often because staff are unsure what constitutes a reportable occurrence or because the organisation's internal reporting procedures are cumbersome. Another common issue is failing to report occurrences within the 72-hour timeframe, particularly when the significance of the occurrence is not immediately recognised.
