What it means in practice
Examinations must be conducted under controlled conditions that ensure the integrity of the examination process. This means adequate supervision, secure handling of examination papers, and an environment free from disturbance. The organisation must have procedures to prevent cheating and to manage examination irregularities when they occur.
The examination process must be documented, including the identity of candidates, the examination papers used, the results, and any incidents that occurred during the examination. Candidates must be positively identified before being permitted to sit an examination.

Examinations must be conducted in a suitable environment with adequate supervision. Examination papers must be kept secure before, during, and after the examination. Candidate identity must be verified. Results must be recorded and retained. The organisation must have procedures for dealing with examination irregularities including suspected cheating.
Common compliance gaps
Inadequate examination security is a frequent finding, including exam papers being stored in unlocked locations or being reused without change across multiple sittings. Another common gap is insufficient supervision during examinations, particularly when large numbers of candidates are being examined with few invigilators.
